Surgical Discharge Checklist

Thank you for selecting Midwest Veterinary Surgery (MVS) for your concierge surgical needs. Please use this checklist to help guide you and your team through discharging your patients. Feel free to contact us with any questions or concerns as they arise.

-Assure all postoperative items have been completed on Day of Surgery Checklist
-Place in patient medical record

Download Discharge Confirmation

Download Discharge instructions from clinic email

-Update Medication section with names and dosages of Go Home meds including time next dose is due
-Print Discharge instructions and also place a copy in patient medical record

Catheter Removal Confirmation

Catheter has been removed

Incision/Bandage Confirmation

Incision/Bandage is clean, dry, intact

Ambulatory/Absent of Pain Confirmation

Patient is Awake, Ambulatory and Absent of pain

Patient is medically ready for Hospital Discharge Verified by_________________ (initials of Attending Vet, Head Vet Tech or Hospital Manager that visually inspected the pet and approved discharge)
